St. Clair Hospital, along with the Washington Health System and two other Western Pennsylvania area hospitals, have affiliated with a Butler County-based home health care provider to help patients make the transition from the hospital to home care.

Concordia Visiting Nurses, a subsidiary of Concordia Lutheran Ministries of Cabot, Butler County, on Sept. 1 will begin helping patients recover at home. Specialty services provided by the Concordia nurses include wound and mental health nursing, physical, occupational and speech therapy, medical social services and home health aides.

“This was a natural fit for us,” said Martin Trettel, chief executive officer of Concordia Visiting Nurses. “It should be a seamless transition.”

As part of its new joint venture with St. Clair Hospital and the Washington Health System, along with Heritage Valley Health System and the Butler Health System, Concordia Visiting Nurses will be hiring 100 to 150 people during the next 18 months as it staffs up in the South Hills, along with opening an office on Bower Hill Road, Trettel said. Overall, Concordia Visiting Nurses is expected to add 250 in the region in that time frame, Trettel added.

Concordia Visiting Nurses is a comprehensive Medicare/Medicaid-certified home health care organization that serves more than 10,000 people annually in their homes. Other services offered by Concordia Visiting Nurses include health technology monitoring equipment, palliative care and spiritual care and disease management.

“St. Clair Hospital and Concordia Visiting Nurses share the same mission and values for ensuring that our patients receive the highest quality of care following discharge from the hospital,” said Dr. G. Alan Yeasted, chief medical officer of St. Clair, in a statement. “This joint venture will provide the right care, in the right setting, at the right time and will prove extremely beneficial to the men and women who entrust us with their health. By integrating with one another and having our clients work side by side, we can enhance the care provided to our patients.”

Gary Weinstein, president and CEO of Washington Health System, echoed Yeasted.

“Washington Health System is pleased to partner with Concordia Visiting Nurses and several excellent community health systems to offer high quality, home-based nursing and rehab services to local residents,” he said.

Under the new partnership arrangement, each of the four hospitals will have a 12 percent stake in the new venture. Concordia Lutheran will be the majority shareholder with 52 percent of the new venture, the company said.
